We are looking for a passionate and enthusiastic team player to join us as a social worker who is interested in making a difference in caring for our residents and clients.
- Conduct psycho-social and financial assessments / reviews.
- Recommend and carry out appropriate clinical interventions and case management for residents and their families.
- Facilitate family conferences and attend multi-disciplinary team meetings.
- Experienced with managing the Integrated Referral Management System and the ILTC portal subvention and subsidy matters.
- Good interperesonal, organisation and communication skills.
- Competent administrative and IT skills in maintaining updated documentation of case files and time reviews of financial assistance
Candidate must minimally hold a Degree in Social Work.
Registered with The Social Work Accreditation and Advisory Board (SWAAB).
Salary Range: $4,000 – $5,500
